Please send any suggestions for this page to the\u00a0<a href=\"mailto:melissa.krasnove@vanderbilt.edu?subject=Epi%20PhD%20Webiste%20\">Sr. Program Manager<\/a>.<\/p>\r\n<p>&nbsp;<\/p>\r\n<h4>Program Handbook<\/h4>\r\n<p>For more details about our program policies and expectations, please feel free to review our <a href=\"https:\/\/medschool.vanderbilt.edu\/epi-phd2026\/wp-content\/uploads\/sites\/122\/2026\/06\/2025-2026-Epi-Student-Handbook-FINAL_v2.pdf\" target=\"_blank\" rel=\"noopener\">Program Handbook<\/a>. <em>(updated October 2025)<\/em><\/p>\r\n<p>&nbsp;<\/p>\r\n<h4>Forms for the PhD Program<\/h4>\r\n<h5>Graduate School Academic Forms<\/h5>\r\n<p>The following forms are used throughout a students doctoral training and are submitted via the <a href=\"https:\/\/vanderbilt.kualibuild.com\/app\/64108a7cf4e2d81965298e12\/run\"><strong>Doctoral Actions<\/strong> <\/a>portal within <a href=\"https:\/\/yes.vanderbilt.edu\">YES<\/a>.<\/p>\r\n<ul>\r\n<li>Appoint or modify the Ph.D. committee<\/li>\r\n<li>Request for Independent Study<\/li>\r\n<li>Schedule or modify the qualifying exam<\/li>\r\n<li>Schedule or modify the dissertation defense<\/li>\r\n<li>Extend deadline to enter candidacy (Qualifying Exam Extension)<\/li>\r\n<li>Extend candidacy (to defend and graduate)<\/li>\r\n<li>Submit the results of the qualifying exam<\/li>\r\n<li>Submit the results of the dissertation defense<\/li>\r\n<\/ul>\r\n<p>More information is available on the Graduate School&#8217;s <a href=\"https:\/\/gradschool.vanderbilt.edu\/academics\/forms\/\">Academic Forms &amp; Guidelines<\/a> webpage.<\/p>\r\n<hr \/>\r\n<h5>Module Request<\/h5>\r\n<p>Students are required to complete 3 modules throughout their training. Use the form below to submit a request for approval by the Director of Graduate Studies<\/p>\r\n<p><a href=\"https:\/\/gradschool.vanderbilt.edu\/academics\/theses\/updated-forms\/Independent-Study-NEW.pdf\" target=\"_blank\" rel=\"noopener\">EPID 8332 Module Request Form<\/a><\/p>\r\n<hr \/>\r\n<h4>Dissertation Resources<\/h4>\r\n<p>The Graduate School outlines the following information on their website regarding dissertation submission:<\/p>\r\n<ol>\r\n<li>For degree conferral at May commencement ceremony and August or December graduate dates, complete the Intent to Graduate Form through the\u00a0<a href=\"https:\/\/yes.vanderbilt.edu\/\">YES Student portal<\/a>.<\/li>\r\n<li>For intra-term degree conferral, complete an\u00a0<a href=\"https:\/\/gradschool.vanderbilt.edu\/forms\/academic_forms\/MARCH30-Application-for-Intraterm-Graduation-20201207.pdf\" target=\"_blank\" rel=\"noopener\">Application for Intra-term Graduation<\/a> for the month you plan to graduate. Deadline dates are listed in the table on the\u00a0<a href=\"https:\/\/gradschool.vanderbilt.edu\/academics\/intraterm-graduation.php\" target=\"_blank\" rel=\"noopener\">Intra-term Graduation website<\/a>. You may choose to walk in the May commencement ceremony that follows your degree conferral.<\/li>\r\n<li>Obtain final approval of your document content from your thesis\/dissertation director and committee members.<\/li>\r\n<li>Follow the Guidelines set forth in the\u00a0<a href=\"http:\/\/gradschool.vanderbilt.edu\/academics\/theses\/index.php\" target=\"_blank\" rel=\"noopener\">Thesis and Dissertation Guidelines<\/a>\u00a0for assistance with formatting.<\/li>\r\n<li>Submit your thesis to\u00a0<a href=\"https:\/\/vireo.library.vanderbilt.edu\/\" target=\"_blank\" rel=\"noopener\">VIREO\u00a0<\/a>for format review. For questions or to request an in person format review, contact\u00a0<a href=\"mailto:%20etdadmin@vanderbilt.edu\" target=\"_blank\" rel=\"noopener\">etdadmin@vanderbilt.edu<\/a>.<\/li>\r\n<li>Following format approval, submit final document via\u00a0<a href=\"https:\/\/vireo.library.vanderbilt.edu\/\" target=\"_blank\" rel=\"noopener\">VIREO<\/a>.<\/li>\r\n<\/ol>\r\n<p>Authors determine the access to their work when creating their VIREO account.
