Use this form to document the temporary export of certain equipment, software, and/or technology under a license exception.
*By filling out and signing the Temporary Export Certification, you are requesting to temporarily export your laptop computer, PDA, cell phone, data storage devices and/or encrypted software. You are also certifying that you will comply with the terms and conditions specified.
**You must carry a copy of this form with you when you travel.
Use this form to document record of a foreign national visiting your department, office, lab, etc.

*This is inclusive of any activity resulting in an export where the information or items leave your physical control (e.g. shipment is sent via a carrier to a non-U.S. destination; emailing of controlled or restricted confidential data).
**It is important to show due diligence in documenting physical exports to foreign countries, parties (individuals and entities), etc.
