Need a form?

1. Please do not save a local copy of these forms. In each instance you need one of the forms located below, please revisit our site to ensure you have the latest copy, as export control regulations are subject to change on a frequent basis.

2. Please read each form in its entirety and fill out completely so as not to delay your request. Once completed, save a copy and email the document to vumc.export@vanderbilt.edu.

3. Keep all export documentation related to each export for five (5) years past the last date of activity regarding your transaction.

Use this form when hiring a foreign national to ensure compliance with I-129 rules and other export control regulations.

International Travel Questionnaire (ITQ) Adobe PDF Icon.png

Use this form to document business-related* travel outside the U.S.

This includes personal travel when Vanderbilt-owned assets will be taken with you.
**It is important to plan ahead and think about what is being exported (laptop, equipment, information, technical data, etc.), where you are going, what entity you are visiting, and with whom you will be meeting, etc.

Temporary Export Certification (TMP) Adobe PDF Icon.png

Use this form to document the temporary export of certain equipment, software, and/or technology under a license exception.
*By filling out and signing the Temporary Export Certification, you are requesting to temporarily export your laptop computer, PDA, cell phone, data storage devices and/or encrypted software. You are also certifying that you will comply with the terms and conditions specified.
**You must carry a copy of this form with you when you travel.

Foreign National Visitor Form Adobe PDF Icon.png

Use this form to document record of a foreign national visiting your department, office, lab, etc.
*It is important to show due diligence in documenting collaborations with foreign parties (individuals and entities).
**Denied Party List (DPL) screens (documenting why they are visiting, what they will have access to, etc.) for each individual, traveler, group, institution and/or entity are necessary. Contact vumc.export@vanderbilt.edu for assistance with running restricted party screening.

International Shipment Review Form Adobe PDF Icon.png

Use this form to document a physical export (of items or information/technology/technical data) outside the U.S.
*This is inclusive of any activity resulting in an export where the information or items leave your physical control (e.g. shipment is sent via a carrier to a non-U.S. destination; emailing of controlled or restricted confidential data).
**It is important to show due diligence in documenting physical exports to foreign countries, parties (individuals and entities), etc.
***Denied Party List (DPL) screens (documenting what and why they are receiving, what they will have access to, what the end-use is of the exported item or information, etc.) for each individual, traveler, group, institution and/or entity are necessary. Contact vumc.export@vanderbilt.edu for assistance with running restricted party screening.

End-Use End-User Statement (EUEUS) Word Logo.png

Use this form to document a physical export (of items or information/technology/technical data) outside the U.S. when there is no intention of the items returning to the U.S. (i.e. donations, hygiene products, certain medical equipment, etc.)

  • This document must be completed by the End-User only, on company letterhead, signed and dated, and returned in PDF format.

*Any permanent export with a value more than $1500 must have a completed EUEUS form. Please contact vumc.export@vanderbilt.edu for additional information before shipping or transferring outside the U.S.

Use this form to assist in the documentation of a physical export (shipment) - when necessary - to an international country.
*Before the invoice is sent, or any payment is made or received, vet each entity via restricted party screening.
