Team seeks to hone in on health care ‘hot spots’

VPIL team received a grant from the Camden Coalition of Healthcare Providers, Primary Care Progress, and the Association of American Medical Colleges (AAMC) to zero in on “super users” in order to identify the reasons behind high utilization and to teach patients how to overcome them.
