Practicum Forms

Year 1 – Spring Term

Finalize your practicum site by February 1.

Year 1 – Summer Term

Report on practicum activities to date. These forms should be submitted half-way through your practicum, and no later than July 25.

• Practicum Progress Report
• Time Record Form (to date)

Year 2 – Fall Term

Upload 5-10 photos from your practicum experience no later than September 1:

Submit the following completed and signed forms within one month of completing your practicum and no later than December 1:

• Practicum Completion Form
• Time Record Form
• Practicum Course Evaluation
• Final practicum document (see Practicum Handbook, page 4)

Site supervisors will receive and return the Site Supervisor Evaluation directly to/from the Practicum Director. The completed form is due December 1.
