Forms and Deadlines

Detailed instructions for practicum deadlines and forms are available on the Brightspace practicum course site.

Year 1 – Spring Term

Finalize your practicum site by February 1.

• Sample Practicum Agreement Form

Year 1 – Summer Term

Report on practicum activities to date. These forms should be submitted half-way through your practicum, and no later than July 25.

• Practicum Progress Report and Time Record (Half-way point or no later than July 25)
• Practicum Completion Form: I
  • Minimum of 5 high-resolution practicum photographs
  • Practicum abstract 

Year 2 – Fall Term

• Practicum Completion Form: II (within one month of completing practicum)
  • Practicum Report 
  • Final Time Record
  • Practicum Products
• Final practicum abstract

Year 2 – Spring Term

• Practicum Colloquium poster

Site supervisors will receive and return the Site Supervisor Evaluation directly to/from the Practicum Director.
